Overview

Lumbar radiculopathy is a condition caused by irritation, compression, or inflammation of one or more nerve roots in the lower back (lumbar or sacral spine), producing pain, altered sensation, and sometimes weakness that travels down the leg in a specific nerve pattern. The lumbar spinal canal contains the cauda equina, a bundle of nerve roots that exit through the foramina just below their numbered vertebrae. Because each nerve root serves a predictable region of the leg, the level involved determines where you feel symptoms: the L5 root, for example, is most often compressed at the L4–L5 disc level. Sciatica, leg pain following the sciatic nerve distribution, is the most familiar form of lumbar radiculopathy.

Lumbar radiculopathy is among the most common causes of spine-related disability worldwide. Symptomatic disc herniation occurs most often between the ages of thirty and fifty, whereas radiculopathy arising primarily from degenerative foraminal and lateral recess stenosis tends to predominate in older adults.

What causes it

The most frequent structural cause is a posterolateral or paracentral disc herniation, in which displaced nucleus pulposus, the soft inner material of a spinal disc, mechanically deforms the adjacent nerve root and triggers a local inflammatory cascade involving cytokines such as tumor necrosis factor alpha. Other mechanisms include foraminal stenosis and lateral recess stenosis arising from degenerative facet joint enlargement, ligamentum flavum thickening, disc-height loss, and spondylolisthesis (one vertebra slipping over another), all of which narrow the channels through which the roots travel.

Both direct mechanical pressure and chemical inflammation contribute to radicular symptoms, which helps explain why some patients improve even when imaging still shows persistent compression.

Recognized risk factors include heavy or repetitive lifting, prolonged sitting or driving with whole-body vibration, obesity, cigarette smoking, sedentary deconditioning, and a genetic predisposition to early disc degeneration. Male sex, taller stature, and occupations involving frequent bending and twisting have also been associated with increased risk. Prior episodes of low back pain or sciatica raise the likelihood of recurrence, and diabetes can compound nerve vulnerability.

Symptoms and warning signs

The hallmark is sharp, burning, or electric pain radiating below the knee in a dermatomal (nerve-territory) pattern, often accompanied by tingling, numbness, and variable weakness. Leg symptoms frequently dominate over any accompanying low back pain. Pain typically worsens with sitting, coughing, sneezing, or straining, and a positive straight leg raise, pain provoked by lifting the straight leg, supports the diagnosis. On examination, findings may include a depressed ankle or knee reflex, focal weakness such as foot drop with an L5 lesion or difficulty pushing up on the toes with an S1 lesion, and sensory loss in the corresponding territory.

The natural history is generally favorable, with most disc-related radiculopathies improving over six to twelve weeks under conservative care.

Certain features require urgent evaluation. Saddle anesthesia (numbness in the groin or inner thighs), new bowel or bladder dysfunction, and bilateral leg symptoms can signal cauda equina syndrome, a surgical emergency. Rapidly progressive or profound leg weakness similarly warrants prompt assessment. Fever, unrelenting night pain, unexplained weight loss, or a history of cancer should raise concern for infection or malignancy.

How it's diagnosed

Diagnosis rests primarily on a thorough history and neurological examination that localizes the affected nerve root. Imaging is reserved to confirm and characterize the lesion when symptoms are severe, persistent beyond several weeks, or accompanied by red flags.

MRI is the preferred study because it best demonstrates disc herniation, foraminal and lateral recess stenosis, and nerve-root impingement without radiation. CT or CT myelography is useful when MRI is contraindicated or when fine bony anatomy must be assessed. Plain radiographs, including flexion–extension views, help evaluate vertebral alignment, spondylolisthesis, and instability.

Because asymptomatic disc abnormalities are extremely common on lumbar MRI, imaging findings must be carefully correlated with your symptoms and examination. Electromyography (EMG) and nerve conduction studies can clarify ambiguous cases and distinguish lumbar radiculopathy from peripheral neuropathy. When the symptomatic level remains uncertain, a selective diagnostic nerve root block may help confirm which root is responsible.

Treatment options

Most patients are managed without surgery. Initial care includes relative activity modification, oral analgesics, nonsteroidal anti-inflammatory drugs, and a structured physical therapy program emphasizing core strengthening, neural mobilization, and education. A short course of oral corticosteroids is sometimes used for severe acute symptoms.

Transforaminal or interlaminar epidural steroid injections can provide meaningful interim relief and help identify the symptomatic level in well-selected patients when performed by experienced proceduralists.

When surgery is considered

Surgery is considered when disabling radicular leg pain persists despite six to twelve weeks of appropriate non-operative treatment, or sooner in the presence of significant or progressive motor weakness. Cauda equina syndrome constitutes a surgical emergency requiring urgent decompression to preserve neurological function.

For a herniated disc, microdiscectomy or endoscopic discectomy are the most common minimally invasive approaches. When radiculopathy arises from foraminal or lateral recess stenosis, particularly in the setting of spondylolisthesis, lumbar decompression, sometimes combined with fusion, may be indicated. Well-chosen surgical candidates often achieve faster relief of leg pain than with continued conservative care, although longer-term functional outcomes for the two approaches tend to converge.
