Lumbar Foraminal Stenosis
Also called narrowed nerve exit in the lower back, pinched nerve where it leaves the spine
A narrowing of the bony exit channel through which a lumbar nerve root leaves the spine, causing sharp, radiating leg pain, numbness, and sometimes weakness.

- Numbness or tingling in the affected leg
- Weakness in specific leg or foot muscles

Overview
Lumbar foraminal stenosis is a narrowing of the neural foramen (the bony exit channel through which a lumbar nerve root leaves the spinal canal) that leads to mechanical compression or irritation of that nerve. Each lumbar foramen is bounded above and below by the pedicles of adjacent vertebrae, in front by the intervertebral disc and vertebral bodies, and behind by the facet joint and ligamentum flavum. Because the exiting nerve occupies a fixed channel, even modest narrowing can produce symptoms, particularly during activities that reduce foraminal volume such as standing, walking, and extending the back. The L4 and L5 nerve roots, at the L4-L5 and L5-S1 levels, are most commonly involved.
What causes it
Foraminal stenosis is primarily a disease of degenerative aging. Prevalence rises substantially after the fifth and sixth decades of life as cumulative disc desiccation, facet arthrosis, and segmental settling progress. Several structural changes contribute to the narrowing: loss of disc height that compresses the vertical dimension of the foramen, posterolateral disc bulging or herniation pressing on the nerve, facet joint overgrowth and bone spur formation, and thickening of the ligamentum flavum. Spondylolisthesis and scoliosis can distort the foramen further, reducing the space available for the nerve.
Risk factors include advancing age, prior disc degeneration or herniation, degenerative or isthmic spondylolisthesis, scoliosis, obesity, smoking, a genetic predisposition to early disc degeneration, and occupations or activities involving repetitive heavy lifting and axial loading. Congenitally small foramina can predispose certain individuals at a younger age. An important clinical point: degenerative foraminal narrowing visible on imaging is common in older adults without symptoms. What distinguishes a patient with lumbar foraminal stenosis from the broader population is whether the narrowing is mechanically compressing a nerve root and producing symptoms that match that level.
Symptoms and warning signs
The typical presentation is one-sided leg pain described as sharp, burning, or electric, radiating down the leg in a dermatomal pattern that corresponds to the affected nerve root, often with tingling or numbness and sometimes with focal weakness. Symptoms classically worsen with standing, walking, and spinal extension (activities that reduce foraminal volume) and improve with sitting or bending forward, which opens the foramen. On examination, a positive straight leg raise or femoral stretch test, reduced reflexes, sensory loss, or focal weakness can help localize the affected level. The natural history is variable: many patients improve or stabilize over time, though some experience persistent or progressive symptoms.
Certain features require urgent evaluation. Numbness in the groin or inner thighs (saddle anesthesia), loss of bladder or bowel control, or weakness in both legs can signal cauda equina syndrome, a surgical emergency that should not wait for a scheduled appointment. Fever, severe night pain, or symptoms in the setting of immunosuppression or recent infection should raise concern for spinal infection. Unexplained weight loss, a known history of cancer, or relentless pain that does not ease with any position warrants assessment for malignancy.
How it's diagnosed
Diagnosis depends on matching the clinical picture to imaging findings. MRI is the preferred study: it visualizes foraminal narrowing, the loss of normal fat tissue surrounding the nerve root, and soft tissue contributors such as disc herniation and ligament thickening, all without radiation. CT scanning provides better detail of bony changes (osteophytes and facet hypertrophy) and is used when MRI is not possible or when precise bony anatomy is needed before surgery. Upright and dynamic (flexion-extension) X-rays assess spinal alignment, disc height, and any sliding of one vertebra on another (spondylolisthesis) that may not be apparent on a supine MRI. Electrodiagnostic studies (EMG and nerve conduction tests) can confirm radiculopathy and distinguish it from peripheral neuropathy.
Because foraminal narrowing on imaging is common in older adults without any symptoms, imaging findings must be interpreted alongside, and must match, the side and level indicated by the clinical examination. Other conditions that can mimic lumbar foraminal stenosis include vascular claudication (leg pain that is relieved by simply standing still rather than by changing posture), hip osteoarthritis (groin pain with restricted hip rotation), and peripheral neuropathy (typically symmetric, stocking-distribution numbness).
Treatment options
In the absence of red flags or significant neurological deficit, treatment begins conservatively. First-line measures include activity modification, a structured physical therapy program emphasizing core stabilization and flexion-based exercises, and nonsteroidal anti-inflammatory medications. For persistent radicular pain, neuropathic agents such as gabapentinoids or duloxetine may be considered. When oral measures are insufficient, transforaminal epidural steroid injections can provide both diagnostic confirmation (verifying that the targeted foramen is the pain source) and temporary relief of symptoms.
When surgery is considered
Surgery is typically discussed when symptoms remain disabling despite a reasonable trial of conservative care, generally six to twelve weeks, or when there is progressive motor weakness or pain that significantly impairs daily function. Operative options center on enlarging the foramen through foraminotomy or decompression. When associated instability, spondylolisthesis, or deformity is present, instrumented fusion is added to stabilize the segment after decompression. Urgent surgical decompression is indicated for cauda equina syndrome or a rapidly progressive neurological deficit. These situations are not managed with a conservative waiting period. Outcomes after well-selected decompression are generally favorable for relief of leg-dominant radicular symptoms.

- Central stenosis narrows the main spinal canal and can simultaneously compress multiple nerve roots or the cauda equina, while foraminal stenosis narrows the side exit channel where a single nerve root leaves the spine. The two can coexist and produce overlapping symptoms, which is why imaging findings must always be interpreted alongside the clinical examination.
- When is surgery considered for lumbar foraminal stenosis?
- Surgery is typically discussed after roughly six to twelve weeks of appropriate conservative care without adequate improvement, when motor weakness is significant or worsening, or when pain is severely disabling.
