Overview

Critical limb ischemia, now more precisely called chronic limb-threatening ischemia (CLTI), is the most severe stage of peripheral arterial disease. It occurs when narrowed or blocked arteries reduce blood flow to the leg or foot so much that the tissue can no longer meet its baseline metabolic needs, even without the added demand of exercise. The result is pain that occurs at rest, and often skin breakdown that will not heal.

This condition is included as a spine mimic because severe leg pain and foot symptoms can sometimes be attributed to lumbar radiculopathy or advanced spinal stenosis before the vascular cause is recognized. Unlike the milder, walking-triggered pain of vascular claudication (covered separately), critical limb ischemia is a true emergency for the limb. Recognizing it quickly and routing the patient to vascular care, rather than down a spine-treatment pathway, can be the difference between saving a limb and losing it.

What causes it

Critical limb ischemia develops when atherosclerotic disease in the arteries of the leg becomes advanced enough, or is affected by an additional acute event such as a clot, that blood flow drops below what resting tissue needs to survive. It represents the far end of the same disease process that causes milder intermittent claudication, though some patients present with critical ischemia without ever having had a prior history of walking-induced leg pain.

The strongest risk factors are the same ones that drive peripheral arterial disease generally: cigarette smoking, diabetes mellitus, hypertension, high cholesterol, and chronic kidney disease. Diabetes deserves particular attention because it accelerates arterial narrowing, commonly affects the smaller arteries below the knee, and often coexists with peripheral neuropathy, which can mask the pain that would otherwise serve as a warning sign. Older age and a personal or family history of cardiovascular disease further raise the risk.

Symptoms and warning signs

The hallmark feature is ischemic rest pain, a burning or aching pain in the foot or toes that occurs even without walking. It classically worsens when the leg is elevated or the patient lies flat in bed, and it improves when the foot is lowered, such as when a person sits up and dangles the foot over the side of the bed or gets up to stand on it. This pattern, pain that tracks with gravity and limb position rather than with spinal posture, is one of the most useful clues that the problem lies in the arteries rather than the spine.

Other findings that point toward a vascular cause include non-healing sores or ulcers on the toes or foot, skin that looks pale, dusky, or bluish, coolness to the touch, and weak or absent pulses at the ankle or behind the knee. By contrast, spinal causes of leg pain more typically produce a dermatomal pattern of numbness, focal weakness tied to a specific nerve root, and pain that changes with posture, sitting, or spinal movement rather than with limb elevation. Sudden, severe leg pain accompanied by a cold, pale limb can signal an acute arterial occlusion and is a true emergency requiring immediate care.

How it's diagnosed

Diagnosis starts with a careful pulse examination and an ankle-brachial index (ABI), a simple bedside test comparing blood pressure at the ankle to the arm. In critical limb ischemia the ABI is typically very low, and additional measures such as toe pressures or toe-brachial index are often needed, since long-standing diabetes and kidney disease can stiffen the arteries and falsely elevate the ABI. Duplex ultrasound, CT angiography, or MR angiography are then used to map exactly where and how severely the arteries are narrowed or blocked, which guides the choice of intervention.

Because leg pain and skin changes have many possible sources, clinicians work to exclude other explanations, including diabetic peripheral neuropathy, spinal stenosis, and radiculopathy. When the history includes rest pain that follows the gravity-dependent pattern described above, along with diminished pulses or non-healing skin changes, the diagnosis points firmly toward the vascular system rather than the spine, and urgent referral to a vascular specialist takes priority over spine imaging or spine-directed treatment.

Treatment options

Because tissue viability is at stake, treatment of critical limb ischemia is considerably more urgent than for milder claudication. Medical management, including antiplatelet therapy, high-intensity statins, blood pressure control, tight glucose control in diabetes, and smoking cessation, remains part of the overall care plan, but it is not sufficient on its own to prevent tissue loss once rest pain or ulceration has developed. Wound care, infection control, and careful offloading of pressure from affected areas of the foot are also central to management, particularly when ulcers are present.

The priority intervention is restoring blood flow to the limb, which is discussed in more detail below. Rehabilitation and long-term risk-factor management continue after revascularization, since patients with critical limb ischemia carry a substantially elevated risk of heart attack and stroke in addition to limb-related complications.

When surgery is considered

Unlike stable vascular claudication, where a supervised exercise trial is attempted first, critical limb ischemia generally warrants prompt revascularization once diagnosed, because delay risks progression to tissue loss or amputation. Depending on the pattern and location of arterial blockage, this may be accomplished through endovascular techniques such as angioplasty and stenting, or through surgical bypass grafting using a vein or synthetic conduit. The choice depends on the extent and location of disease, the patient's overall surgical risk, and available conduit for bypass.

If evaluation instead reveals that a spinal condition, such as severe stenosis or a nerve root compression, is truly responsible for a patient's leg symptoms, decompressive spine surgery becomes the relevant surgical pathway. But this determination should only follow a vascular workup that has ruled out limb-threatening ischemia, given how quickly untreated critical limb ischemia can progress to irreversible tissue loss.

How is critical limb ischemia different from ordinary vascular claudication?
Vascular claudication causes pain only during walking that resolves quickly with rest. Critical limb ischemia is far more advanced: the blood flow is so reduced that pain occurs even at rest, and tissue can begin to break down as ulcers or gangrene. It is a limb-threatening emergency, not just a walking limitation.
Could my rest pain actually be from my spine instead of poor circulation?
Spinal problems can cause leg pain, but they rarely cause pain that worsens specifically when lying down flat and improves when you hang your foot off the bed or stand up. That pattern, especially alongside a cold foot, weak pulses, or a non-healing sore, points strongly toward a vascular cause and needs urgent evaluation.
What happens if critical limb ischemia is not treated quickly?
Without restoring blood flow, tissue loss can progress to gangrene and the limb can become unsalvageable, sometimes requiring amputation. Because the risk to the limb rises quickly, this condition needs prompt evaluation by a vascular specialist rather than a course of conservative spine care.
